Release of liability Waiver
ASSUMPTION OF RISK, RELEASE OF LIABILITY, MEDICAL AUTHORIZATION, AND MEDIA RELEASE
As the parent or legal guardian of the participant, I acknowledge and agree to the following:
Assumption of Risk
I understand that participation in Hi5 Sports LLC programs, leagues, camps, clinics, tournaments, practices, and related activities involves inherent risks, including but not limited to falls, collisions, contact with other participants, equipment-related injuries, weather-related conditions, and other unforeseen hazards that may result in injury, illness, permanent disability, or death. I knowingly and voluntarily assume all risks, both known and unknown, associated with my child’s participation in any Hi5 Sports LLC activity.
I certify that my child is physically capable of participating in the activities offered by Hi5 Sports LLC and has no medical condition that would prevent safe participation unless disclosed in writing to Hi5 Sports LLC prior to participation.
Release of Liability
To the fullest extent permitted by law, I release, waive, discharge, and covenant not to sue Hi5 Sports LLC, its owners, directors, officers, employees, coaches, volunteers, contractors, sponsors, affiliates, facility providers, successors, and assigns (collectively, the “Released Parties”) from any and all claims, demands, causes of action, damages, losses, liabilities, costs, or expenses arising out of or relating to my child’s participation in any Hi5 Sports LLC activity, including those resulting from ordinary negligence of the Released Parties.
I understand that this release does not apply to conduct that constitutes gross negligence, willful misconduct, or any liability that cannot be waived under applicable law.
Medical Authorization
In the event of an emergency, I authorize Hi5 Sports LLC and its representatives to obtain emergency medical treatment for my child if I cannot be reached. I understand that reasonable efforts will be made to contact me or my designated emergency contact prior to treatment whenever possible.
I agree to assume full responsibility for any medical expenses incurred on behalf of my child as a result of participation in Hi5 Sports LLC activities. I further acknowledge that Hi5 Sports LLC does not provide medical or health insurance coverage for participants.
Photo and Video Release
I grant Hi5 Sports LLC the irrevocable right and permission to photograph, videotape, record, and otherwise capture my child’s likeness, image, voice, and participation in activities. I authorize Hi5 Sports LLC to use, reproduce, publish, distribute, display, and promote such materials in any media format, including websites, social media platforms, marketing materials, advertisements, promotional campaigns, and other communications, without compensation or further approval.
I understand that all photographs, videos, recordings, and related materials become the property of Hi5 Sports LLC and may be used for lawful promotional, educational, or business purposes.
Acknowledgment
By registering my child for participation in Hi5 Sports LLC programs and activities, I acknowledge that I have carefully read and fully understand this Assumption of Risk, Release of Liability, Medical Authorization, and Media Release Agreement. I voluntarily agree to its terms on behalf of myself and my child.
